What are the contraindications?
Hypersensitivity to the active principles or to any of the excipients or to other drugs derived from sulfonamide (hydrochlorothiazide is a drug derived from sulfonamide in general, the occurrence of hypersensitivity reactions is more likely in patients with a history of allergy or bronchial asthma. coaprovel is contraindicated in patients with anuria, patients with severe renal impairment (creatinine clearance <30ml / min), refractory hypokalaemia, hypercalcaemia, severe liver failure, biliary cirrhosis and cholestasis. coaprovel is contraindicated in the second and third trimesters of pregnancy. Do not administer coaprovel concomitantly with medicines containing aliskiren in patients with diabetes or moderate to severe renal insufficiency.

This combination medicine is used to treat high blood pressure (hypertension).

PRECAUTION
Before taking irbesartan, tell your doctor or pharmacist if you are allergic to it; or if you have any other allergies. This product may contain inactive ingredients, which can cause allergic reactions or other problems. Irbesartan/Hydrochlorothiazide 300/12.5 mg
What Irbesartan/Hydrochlorothiazide is used for:
It is a combination of Irbesartan, an angiotensin II receptor antagonist, and Hydrochlorothiazide, a thiazide diuretic, indicated for hypertension:
In patients not adequately controlled with monotherapy.  
As initial therapy in patients likely to need multiple drugs to achieve their blood pressure goal.
Do not take Irbesartan/Hydrochlorothiazide if:
Hypersensitivity to Irbesartan or hydrochlorothiazide 
Hypersensitivity to sulfonamide-derived drugs (like sulfamethoxazole/trimethoprim, sulfasalazine)
Anuria present (not enough urine production)
Renal impairment
Liver impairment  
Warnings & Precautions:
Talk to your doctor or pharmacist before taking Irbesartan/Hydrochlorothiazide
To monitor kidney functions 
To monitor potassium levels 
If you have vision problems like myopia or glaucoma
If you have lupus 
If you are at risk to be volume depleted (dehydrated)
Possible Side Effects: 
Fatigue 
Dizziness
Muscle pain
Other medicines and Irbesartan/hydrochlorothiazide:
Tell your doctor or pharmacist about all medicines you take, including prescription and over-the-counter medicines, vitamins and herbal supplements.
Increased risk of kidney damage and reduction of its effects when taken with Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) and COX-2 inhibitors (like ibuprofen, ketoprfen, celecoxib)
Increased risk of lithium toxicity when taken together
Not recommended use with potassium supplements to avoid increase of potassium levels in blood.
With only Hydrochlorothiazide:
Avoid cholestyramine and colestipol to have maximum absorption
Adjust dose of antidiabetics 
Monitor while using alcohol, barbiturates and narcotics because of the potential hypotension
How to take Irbesartan/hydrochlorothiazide:  
Adults above 18 years of age and CrCl >30ml/min: 
The usual dose is: 1 tablet daily 
Irbesartan//hydrochlorothiazide  tablet can be taken with or without food 
It may be administered with other antihypertensive agents 
It can be taken any time of the day 
Do not break the tablet in half 
Correct if known volume depletion prior to starting the medicine

لا تتناول إيربيسارتان/ هيدروكلوروثيازيد:
الحساسية تجاه إربيسارتان أو هيدروكلوروثيازيد
فرط الحساسية للأدوية المشتقة من السلفوناميد (مثل سلفاميثوكسازول / تريميثوبريم ، سلفاسالازين)
انقطاع البول (عدم وجود كمية كافية من البول).
القصور الكلوي
ضعف الكبد
التحذيرات والإحتياطات:

لمراقبة مستويات البوتاسيوم
إذا كنت تعاني من مشاكل في الرؤية مثل قصر النظر أو الجلوكوما

إذا كنت معرضًا لخطر الجفاف
الآثار الجانبية المحتملة: 
إعياء
الدوخة
ألم عضلي
الأدوية الأخرى و إيربيسارتان/ هيدروكلوروثيازيد:
أخبر طبيبك أو الصيدلي عن جميع الأدوية التي تتناولها ، بما في ذلك الأدوية التي تصرف دون وصفة طبية والفيتامينات والمكملات العشبية.
زيادة خطر سمية الليثيوم عند تناولها معًا
زيادة خطر تلف الكلى وتقليل آثاره عند تناوله مع مضادات الالتهاب غير الستيرويدية (NSAIDs) ومثبطات COX-2 (مثل إيبوبروفين ، كيتوبرفين ، سيليكوكسيب)
لا ينصح باستخدامه مع مكملات البوتاسيوم لتجنب ارتفاع مستويات البوتاسيوم في الدم.
